Required Forms:
Registration Form – Complete this form and attach it to the 3
pages of Emergency forms for each student.
Emergency Forms – Child Information Sheet, Release Form and
Policies & Guidelines Form: All 3 forms must be completed, signed,
and attached to the registration form for each student.
First Time Students –
Must also attach the processing fee of $35 (non-refundable) per child
made payable to Moms Day Out.
The
completed forms may be mailed or dropped off to: Moms Day Out, First
Evangelical Free Church, 2223 North Mulford Road, Rockford, IL 61107.
You will
hear about your child’s placement within the next couple weeks once the
forms are received.
